Foundations Health Solutions Selects OnBase by Hyland to Support Post-Acute Care Facilities

OnBase by Hyland platform selected to optimize patient referral management at 64 facilities and streamline HR processes for 8,000 employees

North Olmsted, Ohio-based Foundations Health Solutions has selected OnBase by Hyland hosted in the Hyland Cloud to support its more than 60 post-acute care facilities located throughout Ohio. In addition to leveraging the single information management platform to digitize critical patient information and facilitate preadmission referrals, Foundations Health Solutions will use OnBase workflow functionality to streamline human resources (HR) processes for 8,000 employees.

OnBase will be leveraged to scan, store and access patient information for the 12,000-13,000 admissions at Foundations Health Solutions’ skilled nursing facilities each year. This will eliminate the need to print nearly a thousand pages of patient health information per admission.

Foundations Health Solutions will implement OnBase Workflow and WorkView Case Manager to automate business processes such as insurance verification and medical review. Additionally, OnBase reporting dashboards will provide new reporting capabilities and give an up-to-the-minute view of admissions and business activities.
